PURPOSE

 To implement measures in response to a fatal or critical injury on College property

POLICY

1. The Occupational Health & Safety Act requires that critical injuries and fatalities involving any person at the College shall be reported and investigated.

2.  According to the  Occupational Health & Safety Act, a critical injury is an injury which,

  1. places life in jeopardy;
  2. produces unconsciousness;
  3. results in substantial loss of blood;
  4. involves the fracture of a leg or arm but not a finger or a toe;
  5. involves the amputation of a leg, arm, hand or foot but not a finger or toe;
  6. consists of burns to a major portion of the body; or
  7. causes the loss of sight in an eye.

 3.  When there is doubt if an injury is critical, it will be treated as a critical injury until the Manager, Safety and Security Services or designate has been contacted and has determined its severity.

1.1Contact immediately Emergency Medical Services (EMS) by calling ext. 5000 or 911 or send another person to call.

1.2Call 911 directly to reach EMS at College locations not served by Security Services.

1.3Apply first aid as appropriate.

1.4Take charge of the scene until Security Services or EMS arrive.

1.5Take measures to prevent others from being exposed to the hazards involved in the incident.

Take measures not to move, remove, or disturb any materials, tools, equipment or other articles involved in the incident.

2.1Take control of the accident scene from the first person on the scene.

2.2Take measures to ensure others will not be exposed to hazards involved in the incident.

2.3Assess the level of injury and ensure EMS has been notified, as appropriate.

2.4Contact the Manager, Safety & Security Services or designate and report details of the incident once assessment is completed or within thirty (30) minutes.

2.5Contact the Director or  Dean of the affected area and report the details of the incident.

2.6Control access to the accident scene and ensure potential evidence is not contaminated.

2.7Canvass for potential witnesses.

2.8Escort responding EMS, college officials or other authorized persons to the scene.

Reporting Requirements



3.1Ensure the scene is preserved for the Ministry of Labour inspection, as required.

3.2Obtain the necessary information to carry out notifications required under s. 51 of the Occupational Health & Safety Act.

3.3Notify the President, Vice President of the affected area,   and/or the Director, Advancement,depending on the circumstances.

3.4Inform the Ministry of Labour and the Joint Occupational Health & Safety Committee (JOHSC) of the incident.

3.5Coordinate and begin the investigations by the College and JOHSC members upon their arrival at the scene.

3.6Provide liaison between the College, JOHSC and investigators of authorized agencies involved in the incident

3.7Collect required evidence and information.

3.8Notify the Tragic Event Response Team, if required.

3.9Release the accident scene to the department once all evidence and information has been gathered.

3.10Analyze the observations and information gathered and identify corrective measures in conjunction with the JOHSC.

3.11Prepare recommendations for corrective measures and  submit to the President’s Executive Committee for approval.

3.12Prepare and submiy a report to the Ministry of Labour with the College recommendations.
